I hereby grant permission to Unilabs to perform my COVID 19 PCR screening test which may include obtaining nasal swab sample that can be considered as personal data, and process it accordingly for the testing purpose. I authorize Unilabs to release my result to me through SMS and via the application. In the event of any positive COVID-19 PCR test, I understand and acknowledge that Unilabs is required by law to submit my test result to the Health Authorities in UAE. I agree to take full financial responsibility for the test requested and I understand that payment is required prior to sample collection.

By submitting this form you agree and acknowledge (as relevant)the above  processing of personal and health data.

 

AUTHORIZATION

I authorize my personal data to be processed via the Unilabs ME application .

 

DOCTOR PORTAL

I authorize my exam to be accessed by the doctor who prescribed the exam.
